Did you know that approx. 250,000 deaths every year are caused by medical errors, making this the third-largest cause of death in the US? As with the heartbreaking story of Chris Jerry’s 2-yr-old beautiful Emily (and more recent deaths resulting from unsafe pharmacy practices that caused a meningitis outbreak), the public is often unaware that they are routinely put at risk at medical facilities. Emily’s fatal IV was administered on a Sunday afternoon in 2006. This innocent child died–NOT FROM CANCER, but from an error caused by an untrained Pharmacy Tech. There are still many states that have absolutely no requirements or proof of competency for the pharmacy techs who routinely compound patient’s IV medications on a day-to-day basis.
